Does anyone know how this is triggered or where to look/investigate in the community as I can see no non-approved users and nothing in moderation, it looks like this was added in 12.1.2.21600

That measn based on your moderation options setting for exempting users in a specific reputation percentile, you have more users becoming exempt from abuse than are visiting. Check Moderation Options -> Exemptions(section)-> Exempt Authors from Abuse Automation Minimum Reputation Percentile and potentially adjust as needed, assuming this an issue for your community.
