Ensure that each user has the moderation set in Administration > Membership.
Ensure that the the application is set to moderate, for example, this screenshot of a forum options:
We have two users, both have "Moderated" selected in the Moderation dropdown, but one gets moderated and the other doesn't. Why is that happening? I don't think that selecting every and each application's "Moderate Threads/Replies" to true solves our problem. We don't want to moderate every action within an application, that would give us a lot of headeak; we want to moderate some users actions.
Are they posting in the same application? If the application is not set to moderate, then only posts that are flagged as potentially abusive, get sent to moderation.
I don't find it to be true. They are both posting in the same application, and for one of them the moderation works. Below is an image of the application Options
Are they both members of the same site/group roles?
Yes, the exact same roles, and on the forum they posted, they are both members of the overall group.
Also, I played with the Forum options like you said, I don't know what version you are on but I cannot find the Moderate Threads/Replies checkbox, I only have Moderate Posts checkbox (that only applies to anonymous users), as you can see in the image above. I also checked it to test if you were right, but nothing has changed.

Is it possible the user has a high reputation? Users in the top 10% are exempt by default. Try changing the value to 1 or 0 (disable).
That is true. I changed the value to 0 and now the moderation works for all!
