Previews for uploaded PPT files sometimes generate and sometimes don't. Is this related to the size of the file? Is there a guideline on file size if you want the preview to generate? I've looked in the documentation and don't see that info. Thanks

Any files that fail to provide a preview should have an exception logged, also there is a page in administration (File Viewers > Document Viewers) to see that status of each file.

But why does the issue persist? I have often reloaded the document and still no preview happens. Tried different browsers and various PDFs and it is really a 40/60 split on when the document converts to preview mode and when it does not. thanks in advance

Powerpoint file conversions can sometimes not work because of the contents of the file, they may timeout or they may utilize too much system memory in which case the system aborts. You can adjust the system memory usage in the doc preview plugin but know the more you allocate the more memory consumed in general. If the file has issues, it will probably never work, if you make no adjustments for memory or time, it will continue to fail as well.

The same applies to any supported file type
And without more diagnostic information we cannot really determine any other cause. I would recommend opening a support ticket
